Further surgery following L4/L5 fusion

hi All fellow spine sufferers
13 months ago I had fusion surgery on L4 L5 disc due to degeneration and have never recovered from the sciatic, mainly piriformis pain that I went for the surgery for. I didnt take fusion lightly and exhasted many other options prior to making the decision for surgery.
one year later and talking to a new spinal/Nuero surgeon, he has recomended further surgery on the level below as he is suggesting the same nerve is trapped. This is not a disc, but the wall of the nerve being crushed by loading, mainly driving, which is a large part of my job, driving from one client to another. 
The reason surgery has been recomended is that the same surgeon performed a root block a few weeks ago which the local did infant numb my leg and reduce pain, however came back 2 days later and the steroid never kicked in. Apparently the mri and results from the root block gives my surgeon confidence his procedure will work, I am sceptical not only as I have previously had surgery which basically failed but also I don't want to 'open the flood gates' to numerous surgeries.
any advice from fellow sufferers would be a great help as the advice I currently get is from people who surgery would benefit, but only if it was successfull.
