Post OP back pain

A 75 year old female patient had  herniated lumbar disc surgery (L-4, L-5) 15 months ago. She is presenting with recurrent sever pain (failed back surgery syndrome?) and as of today she has increasing numbness in her toes.She has had one MRI prior to the surgery and two MRI's post op. The doctor that performed the initial surgery declines to perform a second surgery, saying that it is risky. She has seen two more neurosurgeons, both were guarded about the out come. The latest neurosurgeon she saw recommended an EMG (performed 3 days ago). Subsequently the doctor said that based on the results he has advised surgery with no further information on risks and prognosis. We would like advise as to what course to follow. Any comments?
